Dr Tom Hilliar, Dr Danielle Ward, Dr Alison Crofts, Dr Nicole Power, Dr Rhys Holyoak and Dr Jodi Osborn can provide home visits and nursing home visits to patients living in the Kiama Downs are who already attend our practice who are very unwell and to those who have significant chronic conditions that make it difficult to attend the practice. They only make their home visiting care available under extenuating circumstances when patients are unable to access community transport or are, for example, linked to palliative care.
